Could it be the result of insect feeding damage?

yes, it is affected by the aphids, insects, usually you should spray insecticides before harvest.

Insect pest (thrips/aphids/sucking insects) and viral infections may have been involved to cause the fruit deformation of okra fruits, especially when they are grown in nutrient deficient heavier soils.

We have observed such symptoms on okra fruits infested with insects and Tobacco streak virus and Okra yellow vein mosaic virus in India

We also face the same problem in South Gujarat, India. Due to this reason farmers facing huge economic loss, as such fruits graded as lowest grade, so please guide us regarding the same.

It is not related to plant nutrition, but instead to disease or pest effects, check it on the place carefully with specialists
