In recent past we come across three FMD outbreaks in three different regions of North India (Vet. College Ludhiana, IVRI Izatnagar and IVRI Mukteshwar) just after 8 to 10 days of FMD vaccination with IIL Raksha FMD vaccine. Surprisingly FMD was not there in surrounding areas/ villages where vaccination was not done.
In my view, the vaccine used was not safe. Vaccine might be having some not-activated virus which is never determined by the vaccine quality testing and certifying institutions in India.
Because the vaccine is inactiveted .. The problem in the stages of preparation of this vaccine againest FMD, because the survival of any live virus within Stoke may lead to severe outbteak.

It seems that vaccine strains are not inactivated enougly, and therefore it causes infection. You can make genetic and antigenic characterization of field isolates to see differences between vaccine strains and field isolates.
