The SPC/license of fosfomycin in Ireland does not recommend its use in the elderly "due to diminished urinary excretion". What is the experience of its use in other countries, have there been issues regarding poor effectiveness or resistance?
I have used fosfomycin only once in a patient with multi-resistant UTI with an atypical organism sensitive to fosfomycin. This was done only after detailed discussions with the microbiologists and the pharmacist.
We do not routinely use fosfomycin for urinary tract infections.
I use fosfomycin in recurent urinary tract infections also in elderly women, when it is sensible on antibiogram. Generally it is well tolerated apart some intestinal disturbances and well accepted by the patients.
Yes, if you use it as prophylaxis for 3-4 mnths, 1/weekly you can reduce the number of UTI recurrences, I published recently a study on this topic which included patients of different ages, although I didn't analyze the different age-rage but I could see
why would someone want to prophylax the bacteriuria when data has shown that the bacteriuria in the absence of symptoms is protective of future symptomatic bacteriuria
