If you mean blood test, there is no single one for that. Most recent guidelines [like EASL] incorporate composite scores calculated from a number of blood parameters for prognostication like NAFLD fibrosis score, FIB-4, Fibrotest, Hepascore, Fibrometer etc.
The answering of this question appears to be difficult, as there is no single test available to predict those patients with NAFLD who are going to progress to NASH. However, the metabolic syndrome by itself predicts the presence of NASH in patients with NAFLD and probably directs the physicians toward further workup like testing for liver enzymes which reflect the hepatocellular function, particularly the elevated ALT and other noninvasive tests which are helpful in detecting fibrosis such as, FIB-4 (a biochemical formula depends mainly on AST and, platelet count), VCTE and MRI-Elastography ( measure the degree of liver tissue stiffness), etc.
According to three famous American Societies (AASALD, ACG, and AGA), the liver biopsy remains the gold standard for confirmation of unclear cases and exclusion other co-existing liver disease.
AASALD is also advice against the adult screening for NAFLD even in high risk people.
