Interval appendicectomy has been considered obsolete in view of studies quoting the recurrence rate of Appendicitis is too low to warrant surgery for Appendicular mass.
I still recommend a six week appendicectomy in these cases. It is usually an uneventful elective procedure. I do not believe we have overwhelming evidence to break with tradition, just yet.
we had discussion on the same topic about an year ago, please go through it.Majority expressed that is not necessary.I agree with Dr Mohamoud"s discussion.
Thanks for all the inputs (went through the previous discussion, thanks Prof Reddy). The evidence available seems to outweigh the benefits of routine surgical intervention in the form of interval appendicectomy.
Only a minority (about 12%) develop a repeat acute episode.
I do not think the question surrounding interval appendicectomy has been fully answered in kids. All studies I can find, point to the need for a rigorous adequately powered RCT with a long term follow-up. At present we should be informing parents (and the child) of the pros and cons of performing or not performing an interval appendicectomy, and make a combined informed decision.
In our service, usually we perform an interval laparoscopic appendicectomy after 12 weeks from the detection of and appendicular mass caused by an acute appendicitis. Of course, if the parents are agree. By the way, some years ago it was more common, but at this time inflamatory appendicular mass is a rare event. I hope it help.
