What is the relationship between the degree of polynomial fitted in response surface analysis and the number of input and output variables? For example, how many degrees of polynomial should be fitted when there are 5 input variables and 3 output variables in response surface analysis?

It does not depend on the number of variables but on the nature of the investigating process. If your response follows a linear correlation, it might depend on one variable or on five variables. The number of factors does not determine the degree of the polynomial.
