From a basic point of view, AIDS may be defined by increased death rates of peripheral CD4+ T cells and/or the decreased production of CD4+ T cells.
To avoid the former, it might be essential to avoid unnecessary immune activation causing CCR5 up regulation of CD4+ T cells because they are highly susceptible to HIV infection and could also help HIV replication. This might mean that any disease (infectious diseases, cancers, etc) that causes induction of CD4+ T-cell response could be detrimental.
To avoid the latter, it might be necessary to avoid unnecessary turnover of CD4+ T cells, because increased turnover of T-cell progenitors and precursors could cause the reduction of this fraction, or even the reduction of bone marrow CD34+ stem/progenitor cells.
Early ART initiation may be the best way to avoid these problems as much as possible.
AIDS reduces individuals immunity making him/her susceptible to other opportunistic diseases.
