I want to run a survey for scientific research purpose and was wondering what are the minimum identification required from participants?
If the identification information are too specific not much people would want to participate in the survey.
However if its anonymous or too few identifation information given the survey credibility might be questioned.
Hope someone with experience in surveys for scientific research purposes could give me some insights about this. Thanks
Well, while it totally depends on the purpose of your study, you might like to design it something like this -
Name (optional)
Age
Gender
Educational qualifications
Occupation
Job experience {in case, the survey is being conducted on the employees}
This is just an example.

In addition to Kailash's suggestions, I would add this thought. There are usually two primary purposes for collecting personal/demographic information about survey respondents or study participants:
1. To collect information that allows the researcher to describe salient characteristics of the participating sample. Readers of the research may then decide whether the results would or would not be pertinent to their target populations.
2. To evaluate whether such characteristics do or don't help explain differences in responses. Usually, if this were of interest, the researcher would have already declared their interest by having indicated specific research questions (e.g., do perceptions of educational quality differ by the age of the respondent?).
I agree that battering persons over the head with a huge laundry list of requested characteristics is likely to be off-putting to many respondents, and further, will make it more difficult to argue that you are not collecting data in a personally identifiable manner (important for IRB approval, at least in my country).

Thanks for your reply. The survey I'm planning to conduct is to try and establish a relationship between neurodegenerative disorders and other diseases.
The responders personal or demographic info is not of interest to me. The thing is, I'm thinking if I dont have enough participants identifying info. someone might question the credibility of my results. (If those are real participants or just faked up).

I think that the minimum identification to carry out a survey worth its salt lies in making sure that the monitoring of participation is real at all times in terms of identifying the data so that it is of quality, and that problem cases cannot be used for analysis and thus prevent these values from unduly influencing the results. I consider that it is basic to calculate the time required for the response based on the age ranges and it is necessary to note the identification number of the respondent in each analysis.
