When you say iatogrenic-- you mean to suggest that VVF is predominantly caused by medical treatment? I do not believe this is accurate. While it can occur, it is not the most common cause. On page 12, it says: "Obstetric fistula are predominantly caused by a very long, or obstructed, labour... Rarer causes of fistula are from sexual abuse and rape, the complications of unsafe abortions and surgical trauma (most commonly, injury to the bladder at caesarean section). Gynaecological cancers and/or related radiotherapy treatment can also cause this condition, although this is rare in developing countries."
As in the developing world, the main cause of Vesicovaginal Fistula in the developed world is childbirth. It is usually called an "obstetric fistula" in those cases. It can also happen in the event of traumatic and violent sexual assault.
The difference in the developed world is that the majority of women, particularly in urban centres, give birth in hospitals; the likelihood of fistula occurring is reduced, and if it does occur, it can be diagnosed more rapidly.
