I need to sense 5 DC outputs and all outputs have different grounds and is connected directly to loads. I need to isolate the sense points too. I have options like ADC, Analog isolators, Opto's, V2F. But all these will have 5 different power supplies requirements and I need to avoid this. Suggest the best way to do this

You can use an isolation amplifier (like ISO130 ) for each DC output and power its input side (VS1, GND1) from the DC output it is connected.
Then you can power all the isolation amplifiers output side (VS2, GND2) from a common PSU.
Be careful to check the isolation amplifier maximum isolation voltage to be higher than each DC output ground voltage to the PSU ground.

You could use Optocouplers for each DC output and supply the optocouplers output side(collecter,emmiter) from a common supply.
Just make sure the optos operate in a somewhat linear range.

If you want to sence voltages with isolation feature, you must use the different power supplies. For these power supply, MINMAX is a good choice, but it's a little expensive.
I'd used the HCPL7840 before, that is the diffrential optocoupler and it's very acuurate. I think for the output side you can power them with a common power supply and for input side of each voltage sensor you can use the MINMAX power supply.
