A 47-year old male presents to us with metastasis of Leiomyosarcoma in liver. The primary site was the lesser order / upper border of the stomach. The primary lesion was excised, however there was a metastasis to the liver with multiple cyst formation 2 years later.

Since surgical resection of these multiple liver lesions was not possible, the patient was planned for 2 cycles of doxorubicin. Post these two cycles, USG was again taken which showed that the lesions were in stable condition with no regression nor any progression.

Post the primary doxorubicin treatment, the patient was planned for 3 cycles of D-gemcitabine (1400mg) followed by a combination of gemcitabine (1200 mg) with docetaxel (100mg). Post this treatment, USG was taken and compared with the lesions prior to that of chemotherapy. There was no changes seen in the biggest lesion which was yet 35x24 mm in size. A cyst of about 16x17 mm was also seen in right lobe accompanying the main lesion. However, there was a considerable reduction in the number and size of other lesions/masses in the liver. Other organs appeared normal as well.

Reduction in masses showed a ray of hope and the patient was again planned for additional 3 more cycles of gemcitabine and docetaxel of the same dose given before. Till date, 2 cycles has been given already with the 3rd on-going.  Post the cycles, radiofrequency ablation is planned to remove the big lesion.
