What is the best method to analyze the patients satisfaction with scale of national level? Or other suggestions for better running the process.

You can use Likert scale first and then a multiple choice question, items are the same.
Surly, we use Likert scale but different states across the country have different approaches that may can not evaluate satisfaction with same question s. On the other hand if we use customized tool for each state, then we can not compare states, ....
