The diagnostic workup of patent foramen ovale (PFO) provides:

the Transcranial Doppler (TCD) for assess the right-to-left shunt,  the transthoracic echocardiography (TTE) for the diagnosis of intracardiac shunt related to PFO and  the transesophageal echocardiography (TEE) for displaying the PFO and defining all anatomical/functional  features of  interatrial septum and  foramen.

What is the appropriate sequence of these diagnostic tests? the TEE is justified in all cases?
