I have submitted to an Elsevier Journal. The status initially changed from "Submitted to journal" to "With editor" within a day and from "With editor" to "Under review" after 4 days. The status has now changed from "Under review" to "Under editor evaluation" with almost one month in under review. What does it mean?

It most likely means that the evaluation reports of the reviewers have been received by the editor, and the editor is making his/her decision based on those reports.

It means that the editorial team is making their evaluation of the reviewers response/evaluation, and determining the review outcome: accept (hopefully your case; accept with minor revisions; accept after major revisions; invite major revision but revise and resubmit; or reject without invitation for revise-resubmit.
The status "under editor evaluation" indicates that the paper is now with the Associate Editor (AE) who will evaluate it based on the reviews and decide whether the paper should be accepted, rejected, or revised. However, the AE's decision is not final: he/she can only give a recommendation to the Editor-in-chief (EiC) in this matter. The final decision is taken by the EiC. This is when the status changes to "decision in process." Thus, to put it simply, "under editor evaluation" means that the paper is with the AE, while "decision in process" indicates that it is with the EiC.
Dear sir @Gulzar Shah. The status under editor evaluation is same since last 8 days. What may be the reason? Either editor is busy or travelling or they have so many submission?
Dear Asad, for many journals, there is a large pool of associate editors spread all over the globe. Since this is voluntary service the editorial staff sometime send a request to handle the manuscript to an AE, who may delay his/her answer and eventually decline the assignment. Then another one, and another one gets the invite. if this particular journal had a designated handling AE for your manuscript who is busy with his own work, the journal may have given him an extension for week or so, rather than substituting him.
I myself serve as the Special Editor (biostatistic) for one journal which is my high priority. I also serve as Associate Editor for two. For those two, I cannot work on manuscripts for several days.
It means the review comments on your manuscript from experts have been received by the editorial team and they are working on taking a decision on it, whether to accept (with minir/major revisions) or decline the paper for publication based on the review comments.

it mean received the paper from reviewers after evaluation it to Editor for (decision) .

The responses from the reviewers are most likely conflicting and the Editor is looking at their recommemdations to decide on which of them has a stronger/stroNGEST argument. Or in a most likely case when its difficult to decide based on available review, he sends to a third reviewer (who is given s shorter time to review) to give an opinion before the reviewer can conveniently decide.

I think the reviewers have replied their revisions to the editor. Presently, the editor is trying to carefully analyse the revisions, to determine his own decision. Most likely, the editor is not 100% in agreement with the responses, or the responses are conflicting. Hopefully, you will get a positive response soon.
