I agree with you despite of a health system in the country (developed and developing countries) we are lacking in the quality of health services thus health status is poor.

The major reasons I must summarize here -

a. Overgrowing population

b. Rising urbanization

c. Financial constraints

Now, coming to your question the challenges in health system-

a. Lack of human resources to meet the health demand of overgrowing population

b. Poor infrastructure due to lack to financial support from government or other reasons.

c. Lack of political commitment

d. Poor accountability

e. Unequal distribution of health system

f. Rising burden of non communicable disease without a significant fall in communicable disease.

g. Rapid rise of urbanization and inability of health and education system to meet the pace.

Still other reasons there but these are some important areas which I felt are priority.

Thank you for your active participation in this discussion. You mentioned many reasons for the inefficiency of the health system in India. Most of these causes are also present in other developing countries. For example, I live in Iran. Unfortunately, the Iranian health system also faces many challenges. Many of these challenges are similar to your country. Issues such as inappropriate distribution of health resources , inadequate allocation of health funds by government, growth of non-communicable and chronic diseases, over-urbanization, lack of equipment and resources, inappropriate structure of health system , high emphasis on hospital services, inadequate attention on preventive measures, weak management in the health sector. In your country, Is therapeutic services are the priority or preventive services ?

Actually it's a common problem of all developing countries only concern is the magnitude of the challenge. Some countries have less some has more.

We came across Iran health system in an ongoing study on Global Vaccine Study where Iran is also a part. We found the challenges are on same platform as of India.

Therapeutic services always remain priority in Indian system. But since last decade the focus is shifting towards preventive services as you can see the launch of Ayushman Bharat and further enhancement of immunization services.
