In Our region i.e. greater Gwalior and Chambal region of central India incidence of HbsAg (3.51 %) is higher than other parts of India and abroad in healthy voluntary blood donors. We are following the standard blood donor inquiry protocol for selection of donors. Can anyone suggest what the reason was for that?
I guess the answer is in characterizing the patients or the blood donors. as I saw in your paper there is no clear selection criteria mentioned only the partition between male and female was added. To know the reason why you found in your region higher HBV prevalence you should focus on the virus different ways of transmissions, it is very plausible to find one those ways very frequent (for example more exposure to infected blood, or perinatal transmission cases, or the use of infected vaccine is frequent...). Having higher prevalence means you have higher frequency or occurrence of one or many ways of transmission of the virus. As it is mentioned in your paper too the prevalence of HCV in your region is much lower than HBV prevalence even though these two viruses share common ways of transmission, this is probably due to the dominance of the HBV form in your region, which indicates that the transmission of HBV in your region is more mainly insured by perinatal way (from mother to child).

While the voluntary blood donation increased from 44% in 199
7 to 57%in 2007; seropositivity decreased from 1.5% in 1997 to 0.7% in 2007.
That shows that there is some flaw in screening of healthy blood donors in the blood bank and need stict medical screening by an alert medical doctor so that repet/ professional donors are kept away.

The possible explanation is that the area you mentioned has a good proportion of tribal population in whom, it has been shown in India, the HBsAg positivity rate is much higher than general population.

In a study done in Babylon on the epidemiology of HBV infection the prevalence of HBV infection is about 4% during the years 2014 - 2016 and it seems to higher than the years 2003 t0 2007 where the prevalence is around 2.3 %
HBV infection run in families in our study in a study during the years 2014 to 2016 , 6% of case more than 2 members in the same family having HBV infection
