Do they keep throwing all available antibiotics to the patients? Do they eat multiple antibiotics at once or strictly one at a time for a specific period?
And what if all antibiotics failed, what would be the possible treatments? I know phage therapy is on the works, but what about vaccines for these superbugs? Are they here yet, if no then why?

None of the available option can replace antibiotics. The only way is to slow the progression of resistance, discover and develop new antimicrobials and possible modify people behavior towards antibiotics

questions that you mentioned is main concerned for human being for fighting against bacteria in this century.
for first time when penicillin was discovered it can able killed many bacteria but nowaday human being face pathogens that resistant even to last line of therapeutic agent, but speed of discover of new anti bacterial agent reduced.
other issue that you have to consider is that when a new agent discovered it take many times, tests and etc to approving for using in human.
as a researcher that work in this field i hope some day human being win this war and we do not loose anybody.
Give appropriate antibiotic after sensitivity assay, you may still find a suitable antimicrobial for the treatment?
i think because our goal(bacteria or any pathogens) is dynamic system so we should fight with them in dynamic manner. Antibiotics are static so how much we try to find stronger one but at least they will faild against alive nature , so it is better to focus on dynamic solution, it could be phage therapy or teaching our dynamic immune system to fight.
