You can try free option of survey monkey. It is online tool. However, there is limitations of usage in a month.

5. OpinionLab (www.opinionlab.com). Is a voice/means of providing feedback. Has several applications.
6. Crowdsignal (www.crowdsignal.com). collected feedback can be linked with Excel, pdf, CSV, Google Docs.
7.KiyOH. (www.kiyoh.com). can provide feedback on prices and other features and applications.
