We still use alpha blockage with phenoxibenzamine. The dosage should be titrated starting with 10 mg a day, until the patient feels orthostatic hypotension. If he develops tachycardia, a beta blockage should be added. The patient has to be well hydrated. He could start a oral balanced electrolyte fluid as soon as starting the alpha-blockage.
The real danger is during the preoperative period where the Anesthesiologist should be aware of the sudden changes in arterial pressure.
