Data required would include: Depths of aquifer(s), Recharge mechanism, Aquifer media, effects of vadose zone, Hydraulic conductivity of aquifer, land use, Anthropogenic factors, Physico-Chemical & bacteriological characteristics of the water.

You may wish to appy the "DRASTIC" and "GOD" methods of aquifer vulnerability to make your assessment. "Google" these methods for details.

 Should water consumption  and  climate  changes  be included? 

What about surface  water, industry,  agricultural data?

All factors that make the resource vulnerable should be considered, whether surface water or groundwater. Use/consumption pattern and climate change are also significant contributors in assessing water vulnerability.

You could consider the different climate change scenarios as well.
