Mutations and their accumulation are known to cause cancer. On the other hand, there are examples of infections, like H. pylori, which can support cancer development. With better understanding of epigenetic influences in tumor development one could assume, that - at least some - tumor stem cells could evolve not only by initial mutation(s), but then also (theoretically) even only by changes in methylation, or similar, due to some outside changes, perhaps enzymes by bacteria...
