if you do not need a deep degree of investigation, you may use a template provided by Gamma Technologies in its software called GT-suite. there you specify the PHE geometry, boundary conditions (Tin, pin, massflow etc.) and it outputs the PHE performances, temperature profile, pressure drops etc.
the modeling approach followed is one dimensional, whereas flow passages are discretized along the plate length. in each channel, Nusselt based semi-emipirical correlations can be specified according to the application.

I advise to to use Ansys Fluent software, which is very useful software to model heat transfer and CFD in heat exchanger. You can find its Tutorial on youtube.
