I am conducting a baseline audit of a large organizations that provides diverse services to a wide range of clients (housing, employment, etc.). We would like to capture participants perceptions and experiences with services prior to implementing a system wide Trauma Informed Practice intervention. Many of the clients may have low English literacy, significant trauma histories and some have cognitive challenges. I am interested in some quality resources for helping me design the survey instruments for these populations.
Based on the the sample characteristics of being illetrate, I would suggest to use the interview with well trained data collectors.
Then do pilot on similar population to figure out possible pitfalls.

Thanks for your suggestion.. it's not a completely illiterate population. It's a quite divers client population of ~12,500. There does happen to be a range of English literacy levels, and some of the programs serve clients with cognitive disabilities. Some interviews may be feasible, but designing an easy to understand questionnaire to reach as many clients as possible is my main goal.
Have you considered using photovoice, drawing/diagrams, web charts, interviewing. The questionnaire may not work well with all participants but you combine this with some other tools as listed here. The questionnaire could also be administered by trained interviewers - we have done this with large groups where we invited people to a meeting, had food and a discussion and then trained interviewers assisted them to complete the questionnaire. I hope you will be able to consider using strengths approaches alongside trauma informed interventions. Some of our publications listed here on researchgate may be of interest to you.
