Cases of schizophrenia where heritability is absent are not rare. Such patients seem to have psycho-social stressors that distort the perception of reality for them. Such factors have not been narrowly investigated though there is a long history of recognition of such factors in the lives of schizophrenics. Some such factors are family dynamics, complexes, defenses, need for power, personality traits/types, childhood trauma (including PTSD), bullying, marital dissatisfaction/conflicts, social cognition etc.

Please share your views on the factors of psycho-social origin that you believe or have found in your clinical experience that play a major role in causing/contributing to the onset or maintenance of schizophrenia among individuals with no genetic liability.

Its very easy/convenient to say for a clinician that a person has genetic basis (due to positive family history) - in cases where it is so, and probably it happens most of the times - therefore, overlooking the maintenance factors that might be present in the environment or thinking style of the person experiencing those symptoms. This not only effects the therapeutic dedication and responsibility, but also strengthens the belief that schizophrenia and related disorders are incurable and hence very little can be done to improve their conditions in long run. This causes more stigma and disability for schizophrenics, other than life-long sufferings. But more we, as clinicians, recognize psycho-social factors, more it gets possible to bring a shift in our understanding and efforts to help those patients. Not only patients, but their care-givers too. Hence, it becomes more important to atleast understand the factors among genetically non-liable individuals so that a direct relation among such factors and psychosis features can be studied. This can also direct the research efforts where more and more studies need to be conducted investigating the effects of psycho-social factors on psychotic phenomenology. And as clinicians there is a lot to share by us all as we not only assess or recognize such factors but also treat them.

Enlightening comments are expected.
