Female of 10 years of age, without comorbidities, she comes to valoration after 10 days referring to the fall of her height , (she does not know mechanism of injury), she only refers pain to the abduction of the shoulder and to the elevation of the shoulder beyond 60 degrees.
Physical examination: crepitation of the glenohumeral joint, pain in the acromioclavicular joint or clavicle, limitation to abduction in 60-70 degrees, reproducible pain when raising the arm, no rotator cuff tests were performed for the referred pain.
Add shoulder x-ray where partial articulation glenoid continuity solution is observed.
Surgical or conservative treatment?
A short period of rest with sling or arm pouch or plaster slab in severe pain can pretty well help clinical improvement along with pain meds on need basis.
The acute injury is reason for the pain and disability and conservative management will do well here.
As far as any suspected glenoid injury is co concerned , it will heal or remodel uneventfully.
It looks like a glenoid fracture
i would treat for 3 weeks arm sling and then physical therapy
Pain medication as needed.
