It may be a dilemma for coauthorship invitation, especially when a manuscript is completed. Even though authorship should be discussed earlier before manuscript drafting, it may turn into a situation where coauthors can join at last without significant contribution. I would like to gather some opinions on planning coauthorship for different situations including article types (eg research article, review), as a cosupervisor, comments/opinions from experts etc.
Obviously, co-authorship is a tricky, challenging business in that scholars normally have their own concepts of research subjects. As a result their opinions may differ like chalk and cheese and may not be "blended" in the framework of an article or conference paper. I guess co-authorship is only productive and, hence, worth while when a team of lab members present their mutual theoretical input or common practical outcomes. I believe it also works out for a postgraduate student to join their research supervisor in writing an article. Otherwise the co-authors' contribution becomes really imbalanced.
As for joining a completed manuscript, it's absolutely unethical. It's common practice, though.
You cannot add a co-author after a manuscript is completed. I am a reviewer and if I found out, I would not allow it, unless the 'co-author' had been left out by accident AND had made a significant contribution to the paper :)

Andrew Paul McKenzie Pegman I have experience being rejected from my cosupervisor for coauthorship invitation and also me myself as a cosupervisor now that rejected (nor being invited if not related) coauthorship invitation. I would hope to keep a healthy and ethical publication culture.
Forget those guys and publish on your own. They are treacherous and could steal your research. It happened to me :)
As for a supervisor's joining a postgraduate student's manuscript, I'd rather stay away from that sort of collaboration. The thing is that a postgraduate cannot possibly turn down his supervisor's co-authorship, just because of depending on them heavily as long as the doctoral program lasts. Besides, a postgraduate's paper is sure to be immature both in terms of form and content. An experienced researcher normally cares about his record and will hardly join a young scholar's paper, even if they've made a considerable contribution into it. Professors have high standards, you know.
When a professor invites their student to join their manuscript, it's a good thing, I believe. The student does their bit, gains research experience and has a chance to get published in a high-rate periodical which may be unavailable for inexperienced researchers.
I may sound kind of judgemental, but I'll stick to what I started with: co-authoring is a really tricky thing. This is the reason why I prefer working on my own.

But you know when work expands, people may take credit for being a cosupervisor or collaborator. It i s true that people may look for quantity than their record under the pressure of university or in the chase of fame.
Here is my take on this.
You can invite a co-author to join your paper after the first round of revision. Especially when the requested revision by the reviewer is difficult for you to address. You can bring in someone who can help you address the reviewers concerns. That person can be added a co-author in the paper. This is a common practice. It is 100% ethical.

My approach is this:
With my papers/studies, I think the decision is taken jointly by the first author and the last/senior author. We usually have to decide who is "in" and who is "out". It is ideal, as you say, to decide this at the start. But we may make changes along the way if there is need.
