To my understanding, Stephanie is correct on both counts. Two major neurogenic niches have been shown to be significantly active in adults: one feeds the rostral migratory stream and helps you keep your smeller going, and the other feeds into the hippocampus. That said, I believe that much more evidence exists for the plasticity of the system, i.e. existing neurons taking over the function of lost ones, than for out-and-out replacement.
The PNS is a different story from the CNS. Rewiring can and does happen in periphery. This type of thing has not been shown convincingly in CNS, except with experimental manipulation of immune system, glia, chemokines, etc.
It's been thought for really long they were not but recent studies (well maybe it's been few years now) showed that there is stem cells in the adult brain who can proliferate to generate new neurons. I think regeneration is quite limited though. You should look on Pubmed for some specific papers about it.
