A continuing trend towards a decreasing number of MDs in basic science is of concern.
The translational interplay between penetrating science and clinical practice is important; both to keep clinical practice up to date and innovative, but also to increase clinical relevance of Scientific Projects in basic science.
It is pivotal that Medical students have Teachers with clinical background also within pre-clinical subjects.
What are the consequences of this trend?
How can medical doctors be recruited to basic science?
I agree that this is a major concern that probably is subject to geographical variations. In Scandinavia and I believe also in much of western Europe, more MDs are moving away from basic science and you could hardly speak of a trend towards specialization in basic or academic medicine.
Formalizing academic and basic medicine as a specialization could have a positive influence, tempting more young MDs to move into science and teaching. I believe that a negative consequence of not adressing this problem is that future MDs will have less relevant education in basic medical sciences and therefore miss the important translational perspective which connects the clinical presentation of the patient with pathophysiological processes.
I'm interested to know whether academic / basic science is formalized as a specialization somewhere?

There are so many other specialties that motivate the students more and this creates the problem.
