The KAP survey is basically used to gather information on a research topic for the purpose of planning an intervention or a process, implementing and evaluation. It can also be used to know the baseline of a process before adopting a program for intervention or for improvement of an ongoing program.
Simply put, the knowledge assesses respondents understanding of the issue at hand, while attitude as defined by Vaidya (1989) was said to be “a condition of readiness for a certain type of activity” and Anastasi (1990) defined it as, “a tendency to react favourably or unfavourably toward a designated class of stimuli. The practice may be said to be the action/deed carried out to achieve the said process.
To construct the item to be used in measuring each concept, a standardized questionnaire that can be adapted for your research purpose (validity) is required. This you can assess from numerous published papers. To scale for the purpose of analysis requires weighting of the participants’ response (that may be from different categories) to arrive at the dependent variables. This likewise can be assessed from published papers. My advice is to read different articles on KAP for adequate understanding on this issue.

Yes, I think it is not easy to answer a question with this depth and great impact.
But I think that organizing a survey or questionnaire with multiple axes and with questions dealing with these aspects while applying it to a wide segment of the surveyed, then you will have a good indication as an answer to your question ...
This is a wide question, but will try to be as specific as I can. Most researchers I come across are of the opinion that KAP is best handled qualitatively. My published work was on KAP and this was done quantitatively ( Descriptive and Analytical statistics). On this, I will suggest you have standardized data collection tool (questionnaire) and in analysis, the response to your participants must be "weighted" for the purpose of evaluation, for this will eventually be the basis for your analysis.
