Heart block is an abnormal heart rhythm where the heart beats too slowly (bradycardia). In this condition, the electrical signals that tell the heart to contract are partially or totally blocked between the upper chambers (atria) and the lower chambers (ventricles). For this reason, it is also called atrioventricular block (AV block).
Hi , I have used the tablet form of orciprenaline in India, to tide over a period of Heart block when all the pacemakers were busy and my patient had a good basal rhythm. 1 mg/Kg three times a day works well. easy to administer via a feeding tube in children. I do not have the need in adults as we prefer a temporary pacing lead put n by our cardiologists.
The pharmacological tachycardia provides a higher BP than ventricular pacing.
In complete heart block you should nev depend on oral or i/ v beta agonists for enhancing the heart rate. These patients need emergency pacing.
However isoprenaline can be used in atropine sensitive patients to enhanse the heart rate and oral orciprenaline can be used as maintenance therapy for bradicardias other than CHB; after ruling out symptomatic episodic bradicardias.
