Is There Any Documentation of Replication Competent Lentiviruses (RCL)Through Recombination?

It is my understanding that current 3rd and 4th generation gene delivery systems must be tested to ensure they do not give rise to an RCL, before being approved for in vivo use.

Has there ever been a documented case of a generated RCL using these systems? If so, can someone provide a reference(s)?
