In Egypt, where HCV infection is hyperendemic and mostly of genotype 4, we usually test for rs12979860 before initiating peg interferon/ribavirin therapy in patients with chronic hepatitis C. In this study we found a higher prediction for SVR when rs8099917 was genotyped in patients heterozygous for the favorable C allele of rs12979860. Although this adds more cost, TT carriers of rs8099917 showed > 40% SVR if they are allowed to receive therapy.

Interesting finding. The most of previous studies found rs12979860 to be the stronger predictor of SVR than rs8099917. However, I believe that the impact of each SNP should be assessed in different ethnic groups and there is no guarantee to observe same results. This can be as a consequence of different patterns of linkage disequilibrium of these tagged SNPs with the causative SNP (maybe ss469415590) in different ethnic populations.

I agree with Heidar, He is right we should evaluate both in every ethnic group. I hope for easy to treat of variants of IL28b in Egypt (High prevalence of HCV in the world).
