I got intrigued about your question as I have been looking for a Swedish translation for the same questionnaire, but no it is not translated. and it seems it is the same for German. However, you are working at a excellent university so my suggestion is that your department accept the challenge to make the German translation. The questionnaire is not very complicated so it will not be a difficult undertaking for you. Good luck.
