With all the project management methodologies, framework, approaches and best practices in place, why is it so difficult to ensure project success in most organisation?

Perhaps a good project management process requires a good project manager to implement it to the project. However, project management is not mathematics, there is no formula exists to-date to ensure project success. The project manager could follow all the processes, tools and techniques in recommended pm framework (eg. PMBOK, PRINCE2, ICB, P2M etc) to the project but there is no guarantee that the project will deliver within scope, on time, within cost and meet quality. There are too many complexities that affect the project environment being internal or external factors.

So, with a best pm approach together with an experienced and competent project manager, it could only improve the success rate of a project but not guarantee success.
