Hi, my team and I want to publish a paper. However, the journals we are overseeing for possible submission only accept 3 authors and we are 4. Would it be wrong or prone to desk rejection if we submit our paper for consideration with this number of authors?

This is first time I hear about such a rule by a journal, specifically in reference to Medicine which seems to be your specialty. It is very normal for journals and above all Medical journals to accept manuscripts having dozen or even tens of authors, maybe if you disclose the title of that journal we can understand the case better and even investigate why they have chosen to have such rules!
I fully agree with the opinion of Rahim Alijani . Such a rule is completely unrealistic and unacceptable in today's science.
The total number of authors may not be limited by a journal. There are many research projects, especially in medical and biological sciences that require too many researchers working together. However, as we do, while citing the reference in an article, the names of first 6 authors will be written, followed by et al. In my country Pakistan, the regulatory bodies give full authorship credit of an article to first 6 authors and none to the rest.
