My team recently received reviewer comments for a mini narrative review that we wrote basically to say that there is emerging evidence linking a specific intervention/exposure to the disease state that we study, but there is absolutely nothing on a related condition that most people with the disease state we study also suffer from. One of the reviewers rejected because we didn't report on the 'Level of Evidence' which is of the 'utmost importance for reviews'. I have really only seen this done in systematic reviews.

Is it necessary to report the level of evidence in a mini narrative review? I personally feel that doing this would take away from our objective which was to highlight that similar mechanisms drive both conditions so research should be done on this other disease state.
