Hi Shahin, I apologize for my delayed answer: as you wrote the link on BMJ doesn't work, I've searched a lot for the Berlin Questionnaire but I haven't found it, just this reference seems to be the only way to get it :"Fritsche L et al. Do short courses in evidence based medicine improve knowledge and skills? Validation of Berlin questionnaire and before and after study of courses in evidence based medicine. BMJ. 2002;325:1338-41. The questionnaire is available from the authors upon request."
By the way I've found the Fresno EBM questionnaire ( I know it has been validated for MD, nurses and phisycal therapists) ; I send you a second file from PTJ that is interesting, in my opinion. All the best for your research
Does anyone have a copy of the questions? The authors emails bounce and it is not longer at the BMJ site, hence no availability of the questionnaires questions.

Margo Anne Halm I have the same question - I am a librarian trying to find the questionnaire for a faculty member and I cannot get to the main one. The faculty member with the contact information retired in 2020.
