I have edited some dissertation manuscripts that follow the format you describe. Usually there is an introduction chapter, followed by the three articles that have been previously published/prepared. Each article appears independently, with headings and separate citations and references. The only thing is, if we have tables and figures, they should appear consecutively--Table 1, Table 2, etc. That is because readers can get confused if we have more than one Table 1. The final chapter of the manuscript summarizes, synthesizes, and pulls it all together into the theoretical framework and context of existing research. When we cite in APA style, we cite our own articles only if they have been previously published. See the APA style guide, sections 1.10 and 6.02 for some guidance. I hope that helps. Good luck!

Yes. Copy-paste your papers, but edit them heavily for mutual consistency and uniformity and reader-friendliness. Also bring them up to date. For example, by using the keywords in your paper, you may find a new paper that you have to cite. You can even leave in the abstracts, as chapter summaries, or replace each with a "You are here" diagram.

Yes. Quote all authors directly. Secondary references are always undesirable. With your supervisor's permission, you can have references at chapter ends. This reduces the possibility of errors creeping in, and saves time. Your only self-references will be in chapter 1 where you state: "This thesis is based on three published papers (Limaye, 2017a-c)".

Bring it all together in your last chapter, which shows how the whole plan worked out.

Also get your supervisor's permission to number tables, figures and equations including the chapter number. 2-1, 2-2, 3-1, 4-1 instead of 1, 2, 3, 4 etc. Congratulations.

If you are doing just copy paste from your previous publications you are not contributing and bringing anything new. You should cite your original references from your publications and of course use it properly?

The final decision must be made considering your university practices and your supervisors approval of the idea, if the papers were written in a cohenent complimentary way to conform with the subject of the thesis, all is a question of presentation and organization of the written thesis. In my opinion it is better to include the papers as is and add the introduction, discussion and conclusions if this is enough to cover the subject of your thesis proposal. Idealy the decision must have been taken in the framework of the thesis plan before starting to write the papers, so as to integrate them fully in a coherent and natural way. I read many excellent thesis based on a number of publications, one of them was that of the late Prof. Hervé Philip who was one of the members of the examining committee of my thesis, a great Professor who later supervised 8 Excellent thesis.
