I suggest you look at the site of major medical journals. They all have instructions about the structure and the content of manuscripts, including case histories.
Although there many journals for case reports, the vast majority of the journals are very reluctant to accept case report. There reason for this because of its limited value as you can not built up decision on one case.
However the procedure is as follow; abstract ( include summery of the case) introduction, the case and discussion.
Certianly case reports are useful in medicine. As in all branches, medical science is continuosly building process and it is based on information gathered from all types of studies. You will find case report to be of paramount benifit when reviewing rare conditions. As in such situations you will not find enough cohort to be enrolled in other study types.
Many journals are not very interested in publishing such studies, the truth behind this is the low citing rate a case report could get which indeed is not helpfull in growing impact factor a journal may credited, so unless you are reporting a new occurance or a new diagnostic or surgical modality you probably will be faced with a low potential for publication
Writing a case report manuscript will generaly has 3 sections
Introduction: that would be like introduction to any manuscript; reviewing literature and what is known about that condition
Case report: like a case presentation in any scientific meeting; the presenting symptom, history, examination, diagnostic modalities used and how the case(s) was managed. Not that most journals may allow up to 3 cases
Discussion: explain why the rare diagnosis would be appropriate or how this diagnosis is reached. What is the benifit of the new modality or procedure proposed, what and why it is superior to the conventional therapy
Finally, you will have to check regulation of journal you are willing to submitt to. Differences will be in styling, abstract, no of references no of tables and figures.
Even though rare or unique cases are usually recommended for case report, even a common condition with an unusual presentation or good learning points may get accepted.
in my opinion the most important thing is the contribution made with the clinical case, it is important to teach with the clinical cases since it is a great opportunity to learn for those who read these articles
