there are different ways to achieve what you want. Do you have the design module? If not an easy way is by creating the cylinder as a subtraction of two bodies, one the internal cylinder and the other a extrusion of a coaxial parametric curve over a given profile.

In fact , I am using the approach as you are saying. However, the condition is slightly different.

Lets assume this case :

I have a hollow cylinder. Its thickness is 5 mm through out. Now, some arbitrary part of it (very arbitrary , which can not be created by technique like extrusion, as you mentioned) has thickness 3 mm. How can I do it ?

Now, since the thickness of some part is less. In order to get continuous surface, I also need to connect them together.

In that case I would probably use a loft operation, however that is only available in the Design module. Without it you can use a ring body created with a rotation operation as a tool body and perform a Boolean operation (like carving the neck). The ring body can be made of any generic close curve that matches the negative of the shape of the neck in the cylinder.
