Dear Hao Jiang, in the pre-HAART era we used to prescribe Megestrol acetate (brand names: Megestat, in Brazil; Megace, in USA). This drug is used as a paliative treatment in breast cancer pacients with weight loss. It's a syntetic derivative of progesterone. We had seen good results in apetite and weight gain at that time. I would prescribe this drug if other diagnosis are ruled out (TB, neoplasms, ...) and have poor or no choice for a good antiretroviral treatment scheme. I hope this would be of help.
