I am wanting to report a Fisher's Exact as my data violated an assumption for Chi2. Can you please suggest how I report the output from SPSS in the attachment. I appreciate your guidance!

In Fisher's test, only the P value (2-tail) which is known in SPSS as Exact Sig (2-sided) is relevant. So in your case, it is 0.429.
P = 0.429 is not significant as it is greater than 0.05. So, whatever you are comparing, the difference is not significant.
You will report the descriptive information. i.e. n (%) of the groups you are comparing and the p value (0.429).
