I was told we should conduct FGD and IDI to health care providers/patients then do finalise variables in the tool... what all statistical tests i should do to standarize a questionnaire too.... please do give some inputs/reading materials which i can refer too...

Your question is very interesting to read because it is very important for the health care providers and for the patients also. This type of doubts are coming to all the researchers. Good.
If you wants to improve or develop your research questionnaire by focus group discussions with the study participants or patients those who are involving in your study and visiting the facility at the time of interviews with the service providers. An observation based resource availability checklist will also be administer by the researchers. Using by the previous journal papers, find the prevalence of a particular disease based on that you can find the Sample Size for you study.
Ethical clearance also important for this type of study. Written consent form will also be obtain from the study participants. This is also very much important. Don't forget this. You can use the Descriptive Statistics (mean, median, Standard Deviation and proportions) and find the association with the socio-economic status using Chi-Square test and for comparision you can use the t - test in your study.
