How to construct a disconnectivity graph?

I would like to do transition network analysis (and thereafter construct the disconnectivity graph). My system consists of a few peptides that cluster during the course of my simulation. I have used a gromos force field and have computed cluster size with time. How should I proceed further to construct a transition matrix and rate of the association of peptides?
