Have large data sets of LNR using Kaplan Meier Survival curves from term long follow up. Identifying disharmonic changes from in between the LNRs over time may ease to identify cutoff values (and a statistician may give hints going through your results.

I guess I would group the LNR values into 10 to 20 groups and perform an ROC-analysis with the endpoint you are interested in. I would perform this on test cohort and use than the calculted cut-off in validation cohort.
