Sorry I don't know any available questionnaires to measure perceptions of non-native accentedness, but you might find something useful by combing through the literature you find by googling for 'perceptions of foreign accent'.

In terms of statistical analysis, your research question seems to be consistent with a linear regression or hierarchical model, but I'm sure you know that.

If you want to study about their accent, by using a voice recorder you can collect data through conversations. Record both teachers' and students' voices. You better do them individually, the same topic with each student and the teacher. Further, use a questionnaire to identify the student's background and his/her teacher. Later you can compare student's and teacher's accent whether similar or not

Farzad, as a former ESL instructor, I find your research question a very interesting one. While I don't have anything new to add to the previous replies, I noticed that your question included a number of prescriptive grammar errors (I'm a self-confessed 'grammar nerd'!). If you would like my suggestions on rewording anything, I'd be happy to do that. Just let me know if that is something relevant to your purposes.

I would write it this way: I am doing a research project and the question is "Do students' perceptions of the (non)-native-like status of a teacher's accent significantly predict their teacher's credibility evaluations?”

I assume the reference is to an individual teacher, not a group of teachers. If it is more than one teacher, however, the apostrophe would stay after the 's'.
