considering the blood transfusion reactions , what would be the recommendation for the safe rate to start PRBC transfusion during hemodialysis ? our current practice is to be given in 30 min to 1hour and in first 15 min 50ml /hour.
Watch for 5 minutes for transfusion adverse effect , than you can transfuse as fast as required for maintained the critical parameters
Blood transfusion can be given as fast, as possible in present clinical situation
First 3-5 minutes watch for side effects of rapid transfusion

please keep in mind that blood transfusion affects the workload of the patient´s heart a little bit more than saline would. So in order to transfuse "fast", you need to be sure about the pumping function of the patient´s heart, so that you do not decompensate the patient. One hour is a good time in cardially healthy patients, and for the biological compatibility test (or Oehleckers test), it is normally ok to transfuse 30 to 50 ml in about 5 minutes as a start.

it depends on circumstanses .for example as you know if there is severe active bleeding we should have push blood transfusion . but the point is to remove the extra intravascular volume and the elements such as potassium
