I am designing a study that includes a pre-test 11-item scale and a post-test of the same 11-item scale plus open-ended questions. Does anyone know how to set up SurveyMonkey so that we can ensure a match up of respondent answers from the pre-test and the post-test scale? The plan is to import responses into SPSS and each respondent should have their own line.

But, is there a way to do this to ensure their responses are anonymous? If we assign them ID #s (such as with the above embedded link option), we will have to keep a chart matching response ID with subject name in order to send the correct link for the post-test.
This depends heavily on what was approved by your universities' human subjects committee. I would refer to them for requirements and then worry about SurveyMonkey. In our cancer data, we have only an ID number with no information about which patient goes with that ID number. Best wishes, David Booth

I agree with David Eugene Booth , that you'll need to consider what method/s might or might not be approved by an IRB. A lot of the online survey platforms routinely collect an IP address, which can cause problems for assuring anonymity.
One option that would likely pass IRB scrutiny would be to include a field that asked each respondent to create their own, unique ID/alias on each occasion. The logistical problem you encounter is, people can forget or incorrectly recall what ID they used the first time! I have seen this method used with even elementary school students, who appeared to do a very good job of recalling their personal alias.
