In Parkinson's, both internal globus pallidus (GPi) and subthalamic nucleus (STN) are usual targets for DBS electrode placement. I want to know how a doctor decides that for this specific patient, the electrode should e placed in the globus pallidus (GPi) (and not the subthalamic nucleus (STN)). I have the same question about the way of finding the best target in other disorders like OCD.
In Parkinson's disease patients in which the tremor is predominant, The STN is best target. while in Parkinson's patients with predominant rigidity then the GPi is the best target
Both STN & GPI DBS are effective for PD. STN-DBS is considered when the aim is to reduce medications and for patient with tremor. In contrast, GPI-DBS is considered for patient with cognitive decline concerns.
