A lady presented with headache and dull mood 15 days after taking oral retinoic acid for icthyosis. Her MRI brain was s/o demyelintion in the bilateral parietal and frontal lobe with sparing of cllososeptal area.
Lady was 40 yrs old. There was no comorbidities except icthyosis. she was taking oral isotretoin(5) for this.Her fundus was also normal.csf showed slightly raised protein with normal cell count. I have gone through literature, and found some case report of reversible localized demyelination of cerebellum, but there is no report of diffuse demyelination. i stopped the medicine and and after a week she noticed some improvement in her symptoms. my plan is to follow the patient.please give ur valuable advice.
sounds interesting. let's follow her with repeat imaging after a month time and look for any resolution, if that is there make it a case report. and let me know plz
